    Corporate Identification

  • Corporate name exactly as registered
  • California Secretary of State file number (7 digits)
  • Addresses

  • Principal Executive Office: Where your highest-level executives work. Can be outside California.
  • Mailing Address: Where you want to receive correspondence
  • California Office: Physical California address, if you have one (can write "NONE" if you don't)
  • Officers and Directors

    You must list:

  • Chief Executive Officer (CEO): Name and complete address
  • Secretary: Name and complete address
  • Chief Financial Officer (CFO): Name and complete address
  • All Directors: Names and complete addresses
  • Notes:

  • One person can hold multiple officer positions
  • Corporations must have at least one director
  • California corporations with 3 or more shareholders must have at least 3 directors
  • Agent for Service of Process

  • Agent name (individual or corporation)
  • California street address (no P.O. boxes)
  • City, state, zip code

    1. Go to bizfileOnline.sos.ca.gov

    2. Create an account or log in

    3. Select "Statement of Information"

    4. Enter your corporate name or file number

    5. Complete all required fields
